We can generate superbills for you to submit to your insurance company to seek reimbursement for these services. We cannot guarantee that your insurance will cover these costs and each company has different policies for reimbursement. In order to make superbills, we will need a diagnostic code from your physician or therapist.
The No Surprises Act requires all healthcare providers to provide clients estimates for the costs of their care both orally and in witting. The Good Faith Estimate shows the cost of items and services that are reasonably expected for your healthcare needs and treatment. This will be provided by this office upon scheduling and/or as requested. This Good Faith Estimate does not include unexpected costs that could arise during treatment.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. However, this practice cannot predict the need for hospitalization, medications, and medical tests, which will be ordered by other clinicians. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

Any appointment that is not cancelled or rescheduled within 24 hours of the appointment will be charged for the full amount.
All payments are expected at the time of service. Credit cards, debit cards, HSA/FSA , checks, and cash are accepted. No insurance plans are accepted at this time. However, your insurance company may provide benefits for nutrition services by a licensed registered dietitian. It is the responsibility of the client to pursue reimbursement. All appropriate forms are available to help you upon request. Call your insurance company for more information.
If paying by check, please note that there is a fee of $50.00 for any checks that do not clear.
